I hadn't originally intended to scan this, but life dealt me a sharp reminder recently of how fragile material possessions truly are, so I decided to make an electronic copy of it, just in case something ever did happen to the original. And, if I was going to scan it anyway, I didn't see any reason not to put it up on the site while I was at it...
